Patient Rights & Responsibilities

As a patient of Lighthouse Anesthesia, PC, you have the right to:

  • Be treated with respect, dignity, and consideration by competent personnel;
  • Know the name(s) of your Physician or CRNA responsible for your care;
  • Receive informed consent prior to the procedure to make an informed decision regarding your care and treatment plan;
  • Receive truthful information in advanced about the risks and benefits of your care process;
  • Participate in your health care decisions and the ability to refuse care or treatment;
  • Personal privacy, security, and confidentiality of your medical records;
  • Be free of neglect, abuse, or exploitation;
  • Receive services without discrimination of race, color, religion, gender, orientation, national origin, or payer;
  • To understand the fees and payment policies involved in your care;
  • Understand all records and communication will be treated confidential, and to approve or refuse their release except when release is required by law;
  • Ability to obtain access to information contained in your medical record within a reasonable time period when requested;
  • Voice grievances or complaints about your services without discrimination or reprisal and have those issues addressed.

As a patient of Lighthouse Anesthesia, PC, you are responsible for:

  • Providing the most accurate and complete information to the best of your knowledge regarding your health history, medications, unexpected changes, or any other health matters;
  • Read and understand all permits and consents to be signed: if you have questions make sure to obtain clarification from your nurses or providers prior to signing documents and consenting to care;
  • Follow the treatment plan instructions for the entire course of treatment, including instructions given prior to and post procedure date;
  • Promptly fulfill financial obligations for charges of services provided by Lighthouse Anesthesia, PC;
  • Inform your provider regarding living wills, advanced directives, medical power of attorney that may affect your care;
  • Be respectful of all staff, health care personnel, providers, as well as other patients, visitors, and billing staff.
